PE / Extra Curricular Activities Release Form

Premier Academy offers a physical education class to fulfill the Illinois State Board of Education’s physical education requirement. PE is conducted at the Premier Academy gymnasium Monday-Friday.

Physical Education does involve a cost to students; payment is due upon enrollment.

Physical Education IS NOT OPTIONAL – it is mandated by law. The only students who will be excused from PE are those who have a PE waiver from their doctor.

Students may be asked to participate in various activities, which include but are not limited to: PG13/R movies, photographs, interviews, and letter-writing campaigns for Premier Academy. Participation in any of these activities would require a signed permission form from the parents. Please sign below and indicate whether or not you allow your son or daughter to participate.
